
Do I Need a Referral for Concussion Physiotherapy?
The short answer: no. You can book directly, right now. Here's how it works in Ontario.
No referral needed — you can self-refer
In Ontario, physiotherapists are primary healthcare providers. This means you have the legal right to see a physiotherapist directly, without a doctor's referral, for any condition — including concussion.
You don't need to see your family doctor first. You don't need to go to a walk-in clinic. You don't need a note, a form, or an authorization from anyone. You can simply book an assessment and come in.
This is called direct access, and it's been the standard in Ontario physiotherapy for years. Most people just don't know about it.
Why this matters for concussion recovery
The referral requirement is one of the biggest barriers to timely concussion care. Here's what typically happens when a referral is required:
- You call your family doctor — earliest appointment is 1–2 weeks out
- The doctor confirms the concussion and writes a referral — you've now lost 2 weeks
- You call the physiotherapy clinic and wait for another opening — possibly another week
- By the time treatment starts, you're 3+ weeks post-injury
Meanwhile, current evidence clearly shows that early intervention leads to faster recovery. Every week of delay is a week of missed opportunity for targeted treatment.
Direct access eliminates this barrier. You can be assessed within days of your injury — often the same week.
What about insurance coverage?
Most extended health insurance plans cover physiotherapy regardless of whether you have a referral. You typically just need the receipt from a registered physiotherapist.
That said, a small number of plans require a physician referral for coverage. If you're unsure, check your benefits booklet or call your insurer before your first visit. We can help you navigate this.
Important: Even if your insurance requires a referral for coverage, you don't need one for access. You can always see a physiotherapist directly — the referral question is just about who pays.
What about OHIP?
Physiotherapy is not covered by OHIP for most working-age adults. OHIP does cover physiotherapy for individuals under 20, over 65, or on Ontario Works / ODSP. For everyone else, coverage comes through extended health benefits or self-pay.
What about motor vehicle accident (MVA) injuries?
If your concussion was caused by a car accident, your treatment may be covered through your auto insurance. In most cases, auto insurers do require a physician referral or a treatment plan from the treating physiotherapist. We can guide you through this process.
Can a physiotherapist actually help with concussion?
Yes. Physiotherapists with advanced training in concussion management are among the most qualified professionals to assess and treat concussion symptoms. This includes:
- Vestibular rehabilitation for dizziness and balance problems
- Cervical spine treatment for headaches and neck pain
- Oculomotor retraining for screen intolerance and visual processing issues
- Graded exercise therapy for exertion intolerance
- Return-to-sport and return-to-work programming
